Have you used a local primary care service for any unplanned treatment (unplanned treatment includes that arranged for illness, accidents, new conditions, pain, etc.)? This includes a pharmacy, GP, calling 111 or using a local hospital.

Healthwatch East Riding of Yorkshire, which works to help local people get the most out of their local health and social care services, is running an Appropriate Service Review Survey.

The survey will benefit those who take part, as it allows for views to be expressed on services and will ensure that any opinions are heard.

The data collected from the survey will also help to better the feedback given to local services, which should help these services to improve.

The survey can be found at www.surveymonkey.co.uk/r/hweryservicereview and should be completed by Saturday, 30 November.
